Please complete form for your student absence. Doctor notes must be dropped off in the main office or emailed to Ms. Lopez
Notes received more than 30 days after absence will not be excused. Stop by the office with any questions or email Ms. Lopez (janet.lopez@k12.dc.gov).
Attendance Policies: Parents have 5 days to submit excuse notes and/or doctor's notes. Doctor's notes will not be accepted past the 5-day window. Doctor's notes are required after 5 cumulative days within the academic term.
